Uncertain Smile heette aanvankelijk Cold Spell Ahead en werd opgenomen in 1982 in New York met Mike Thorne als producer, nadat Matt Johnson een contract bij Epic Records had verkregen voor The The. De relatie tussen Johnson en Thorne verslechterde echter snel als gevolg van Johnson's drank- en drugsgebruik en meningsverschillen tussen hen beiden over de productie van de nummers. De sessies werden uiteindelijk afgebroken en Johnson keerde terug naar Londen.

Hij begon met opnemen met Thorne's voormalige ingenieur Paul Hardiman, en arrangeerde zo de uiteindelijke single. De singleversie van Uncertain Smile had fluiten en een saxofoonsolo van de Uptown Horns-oprichter Crispin Cioè, maar voor het album werd het nummer opnieuw opgenomen. Ter vervanging van de saxsolo werd een lange pianosolo van Jools Holland geïntroduceerd.
